Amazon Product Feedback email not Activating

Hi there, I've created an Amazon Product Feedback email, set the parameters, the sender email is approved in Seller Central, but when I click Activate, I'm taken to a blank page. And when I go back to the Amazon Feedback Mgmt page, the review request email is still in Draft mode. Screenshot below. Not sure what I'm missing? Thanks!
