It could be that I don't understand the intended behavior of auto refill, but how does auto refill work when setting the minimum balance at $0?
From my experience, the only way the postage will refill when setting min balance @ $0 is if when drawing down the postage balance, it needs to exactly hit $0 to have the auto refill kick-in. However, when printing labels, say the label costs $1.93 and the remaining balance in my account is $1.35, then a modal dialog will appear asking me to manually refill with a specified amount. It seems like the application won't recognize the balance will be negative with the purchase of the label and proceed with the auto refill.
Is this the intended behavior or is this a bug? I realize I can set the minimum balance above $0, but for my first-class account, I prefer to draw it down all the way to $0 instead of carrying a balance like my priority account.
Also, I have noticed a similar issue occur when setting the auto refill minimum balance above $0 and making a purchase that exceeds my balance. For example, say my balance is $60, my auto refill is set at $50, and I'm making a batch purchase of labels with a total postage cost of $75. In this instance, the purchase exceeds the funds in my account, but my account balance is not low enough to kick-in the auto-refill feature. So I get presented with the modal dialog asking me to manually refill my account. This occurs less often, but it does happen on occasion.

There appears to be a few things happening here.
Ideally, according to your process (specifically for First Class postage), you would prefer to pay per use for your labels. Unfortunately, Endicia does not allow this, but as we are constantly in communication with Endicia, I suggest you place a feature request so you and others can be notified if and when this option this is released. For now, you will likely have a balance remaining on your account.
Setting the refill amount to zero can be problematic, because you would have to reach a zero balance before the amount would refill. I suggest you place the value a bit higher so this is triggered. We will not trigger it mid-label. Per your example, if you are trying to purchase a label for $1.93 and your balance is $1.35, but your minimum balance is 0, the auto-refill will not be initiated in the middle of your label purchase. We are always looking to improve and you could place a feature request for that as well!

Regarding your batches, if you have auto-refill enabled, it should purchase additional postage mid- batch if the cost of the batch exceeds the amount in your account.
Per your example:
Cost of your batch is $75
Current balance is $65
Minimum balance is $60
Auto-refill is set to add $50
If you attempt to purchase the full batch, the auto-refill should be triggered partway through the batch to add $50 to the account, and you should not have to manually select to add postage.
Please verify your minimum balance and auto-refill settings (make sure to select these settings same account as the batch for the postage purchase attempt, ie Expedited or First Class).
