Postage paid for twice? Customer had to pay to receive her package - now I have to refund her

Hi! Can you please help me get some support for this issue?
 
A package I sent to a client was bypassed by the post office.
They insisted she come to the PO to pick it up AND pay over $8 for the shipping.
 
I now need to refund her for the shipping.
But I'm confused as to how this happened. 
I paid for and printed the label using ShippingEasy.
