Add "Required Shipping Method" to Order Upload CSV

Currently when uploading a CSV the system doesn't have a field for Required Shipping Method which is very important.

Simply ad this field to the CSV.

Support for mapping a field in a CSV file to the Requested Service for an order in ShippingEasy was added on June 11, 2014.
