File a UPS One Balance Insurance Claim | How To

All UPS from ShippingEasy One Balance shipments include a maximum liability coverage of $100 for all shipments at no cost, known as UPS Declared Value.  You have the option of declaring a higher value of the item at the time of the transaction and will pay an additional charge to cover the amount declared.

To submit an insurance claim request for a UPS One Balance Shipment

To start your UPS from ShippingEasy One Balance claim for a lost or damaged package, complete the form below. To avoid any delays in processing your claim, please provide all required information and include photos of damaged item (if applicable).

IMPORTANT: This form is for Lost or Damaged Parcels shipped through the UPS from ShippingEasy One Balance only. If your parcel was shipped under a UPS account opened outside of ShippingEasy, go to UPS.com to file your claim.

Claims requirements:

  • All claims must be submitted within 60 days after the shipment date.
  • Claims can take up to 3 weeks to be processed.
  • If a claim is denied, you may appeal by submitting a new claim with new or updated information.
  • Claims with partially filled claims form cannot be processed. If there is an issue with your submitted claims form, you will be notified via email.
In order for this form to be submitted, all fields must have an entry. If they do not apply to you claim, please enter N/A into the field.

Loss Claim: Complete this section if package was lost:

You can file a claim on a package if it has not been delivered 24 hours after the expected delivery date and time.

Before filing a claim, be sure the recipient has checked all exterior doors and other locations where the package could be placed, including the porch, back patio, garage, and any area out of potential weather hazards. Also, check with anyone who might have retrieved the package.

If still unable to locate the package, the shipper must initiate the claim.

Describe the contents of the package. Include size, brand, color, model number, etc.



Damage Claim: Complete this section if package was damaged:

The shipper must gather the needed information from the receiver and initiate a claim on a damaged package.
